Before filling out your application, please review our CNA Program checklist to ensure you have all the necessary documents and forms. Click the link below to download a copy of the checklist (PDF).

- Download, print, and fill out the physical exam and immunization forms (pages 1-7):
- Pages 1-2: To be completed by you.
- Pages 3-4: To be completed by your healthcare provider/doctor (physical exam).
- Pages 5-7: To be completed by your healthcare provider/doctor (immunizations, TB test results, and flu shot form).
- Submit the following with your CNA application packet:
- Completed physical exam and immunization forms (pages 1-7).
- A copy of your immunization records and/or titers (lab work) for MMR, Hep B, Varicella, TDAP, COVID-19, flu shot, and TB test results.
PROGRAM DETAIL
A certified nursing assistant, or CNA, plays a crucial role in the healthcare system by providing essential care to patients under the supervision of a Registered Nurse (RN) or a Licensed Vocational Nurse (LVN).
AAHS, we understand the diverse settings in which CNAs are needed. Our graduates are prepared to work in various environments including nursing homes, hospitals, adult day care centers, personal homes, and assisted living facilities. CNAs serve as the vital link between patients and the healthcare team, ensuring continuity and quality of care.
CNAs from AAHS are trained to meet the basic quality-of-life needs of patients of all ages. In residential nursing care facilities or outpatient clinics, our CNAs are equipped to handle a variety of tasks, from daily living activities to monitoring patients’ conditions. This role requires not only technical skills but also compassion and dedication, which we instill in all our students.
Given their daily interactions with patients, CNAs are pivotal in gathering critical information about patients’ health and well-being. This information is crucial for RNs and LVNs to make informed decisions about patient care. What should I bring on registration/orientation day?
Please bring a government-issued ID, proof of education, initial payment, immunization records, background check documentation, and any completed forms sent prior to orientation.
What is included in the tuition cost?
The tuition cost includes textbooks, study materials, uniforms, necessary supplies (e.g., stethoscope, blood pressure cuff), lab fees, exam fees, and an ID badge.
How long is the CNA program?
The duration of our CNA program is typically 4 weeks to 2 months, including both classroom instruction and clinical training.
What are the prerequisites for enrolling in the CNA program?
Applicants must be at least 16 years old, possess a high school diploma or GED equivalent, pass a criminal background check, provide proof of immunizations and a negative TB test, and demonstrate proficiency in English.
